What are the Adverse Effects of Testosterone Replacement?

Testosterone is a growth hormone. The most common adverse effect is an increase in the red blood cell mass, which is called polycythemia. If this occurs, blood donation at the Red Cross is the treatment.

In the past, testosterone was the source of controversy. Previously, some physicians believed that testosterone replacement is associated with increased risk of myocardial infarction. After substantial research published in The New England Journal of Medicine, that view has not been supported. For decades, urologists believed that testosterone promoted prostatic cancer. Through the persistent pioneering work of Abraham Morgentaler MD at Harvard, the view that testosterone promoted prostate cancer has been discredited.

There is no moral superiority in testosterone deficiency. There is no requirement for graduation or discontinuation. My focus is long-term stability.

Why is Testosterone Replacement Important for Men on Suboxone?

In my Suboxone practice in Nashville, I offer testosterone replacement. Many recipients work in the trades or construction, often 50-70 hours per week.

Physical strength and endurance matter. Being strong is important for a man, particularly in Nashville’s booming, but competitive economy. On each visit, we focus on how a man is functioning at work and in daily life.

Suboxone may suppress testosterone. Every man on suboxone thereapy via my Nashville suboxone clinic is at risk for testosterone deficiency. If a patient on Suboxone has symptoms of testosterone deficiency, we discuss whether the evaluation and treatment align with goals and preferences.

How We Approach Testosterone at Our Practice

If you have symptoms of testosterone deficiency which are bothersome to you, then you will obtain laboratory results to confirm low testosterone. This is done once. Your baseline CBC and PSA will also be established.

Testosterone can be given orally, by injection, or by application of a gel. The most-effective, least-expensive treatment is injection. I teach the patient the injection technique while providing a specific prescription for needles and syringes. My patients easily master the technique of home injection.

The prescribed dose is physiological, not pharmacologic. I do not provide testosterone for the purpose of body building.
